چکیده

PURPOSE OF REVIEW Central neuropathic pain is an important and disabling but often neglected problem following central nervous system lesions. The present review highlights recent advances in the understanding of the underlying mechanisms and in the diagnosis and treatment of central pain. RECENT FINDINGS Within the past year, the field of central pain has moved toward a more integrative understanding of central pain. The involvement of nonneuronal cells and interaction between multiple areas of the central nervous system has been recognized as important in the underlying mechanisms. The interest for conditions other than spinal cord injury, multiple sclerosis, and stroke has increased, and continued discussions on clear clinical diagnostic criteria are needed. The treatment of central pain is still a great challenge, but recent evidence points to tricyclic antidepressants, gabapentin and pregabalin, and selective serotonin-noradrenaline reuptake inhibitors as first-line drugs in central pain. An increased understanding of the psychosocial aspects of central pain also has implications for the treatment. SUMMARY Increased insight into the mechanisms of central pain will hopefully lead to increased efforts to study mechanism-based treatment of central pain.

Medicinal herbs in the treatment of neuropathic pain: a review

Chronic neuropathic pain is a common significant and debilitating problem that presents a major challenge to health-care. Despite the large number of available drugs, there are no curative conventional treatments for neuropathic pain. Nowadays, more attention has been focused on the herbal formulation in the field of drug discovery. Therefore, we performed an extensive review about herbal drugs...

The effect of nimesulide on CoxII expression in central and peripheral immune cells (microglia and macrophage) in a rat model of neuropathic pain

Introduction: Neuropathic pain may be due to a primary insult to the peripheral or central nervous system. In this situation, Hyperalgesia and Allodynia are the results of prostaglandins and cytokines release in the spinal cord. It seems that immune cells play an importat role in the induction and maintenance of chronic pain. Compared to selective CoxII inhibitors, nimesulide, a highly select...

The effect of chronic caffeine administration on hyperalgesia in a rat neuropathic pain model: role of nitric oxide pathway

Background: Neuropathic pain is a chronic pain caused by damage to the central nervous system and the peripheral. Caffeine is a non-selective antagonist of A1, A2a, receptors of adenosine, which has a protective effect on neuropathic pain in some doses by inhibiting A2a, A2b receptors. Considering that the nitric oxide (NO) levels are apparently effective in the parts of caffeine central effect...

Effects of combined chronic progesterone administration and forced exercise on behavioral pain responses in the neuropathic pain model of chronic constriction injury in rats

Introduction: Neuropathic pain is a chronic pain that results from damage to the central and peripheral nerves. According to the previous studies, physical activity or progesterone can be an effective treatment for alleviating sensory neuropathic pain individually. In this way, the combined effect of chronic progesterone and forced exercise on behavioral pain responses in the neuropathic pain m...

Effects of co-administration of chronic curcumin and forced exercise on behavioral pain responses in the neuropathic pain model of chronic constriction injury in rats

Introduction: Neuropathic pain is a chronic pain that results from damage to the central and peripheral nerves. According to the previous studies, curcumin and exercise can be effective treatments for alleviating sensory neuropathic pain individually. Now the combined effect of chronic curcumin (Cur) and forced exercise on behavioral pain responses in the neuropathic pain model of chronic const...
